This is an informational medical website. When you submit the appointment form, the scheduling details you entered are sent to the office by email and are stored so the request is not lost in transit. The form collects contact and scheduling information only.
The form collects your name, phone number, email address, a preferred date and office, and whether the condition may be work-related. It does not provide a field for symptoms, diagnosis, treatment history, or other medical details, and you should not add them. This website and email are not for emergencies. Call 911 or seek immediate medical care for an emergency.
Advertising measurement
This site runs no advertising or social-media tracking pixels on any page. If you arrive from a search advertisement, the advertising click identifier that Google appends to the web address is stored in a first-party cookie on your device and submitted with the appointment form. It is used only to record that an advertisement produced an appointment request.
That record is reported to Google Ads from our server. Google is not given the pages you viewed, the conditions or procedures you read about, or any information you entered in the form. If you never submit the form, nothing about your visit is reported to any advertising service.
Website traffic is measured with a self-hosted, cookieless analytics tool that records page addresses and general traffic patterns without identifying individual visitors and without sharing data with an advertising network.
Website measurement
Session-recording analytics are disabled on this website while medical-privacy requirements are reviewed. Standard hosting and network systems may still process technical request data needed to deliver and secure the site.
Do not use the appointment form or email to submit sensitive medical information.

You may browse the informational pages without using the scheduling-email form. The current site does not set analytics cookies or run session-recording analytics.
